| Video Discription | This video demonstrates an internal maxillary artery (IMA) to M2 middle cerebral artery (M2) bypass with a modified superficial temporal artery (STA) graft. This surgical anatomy video provides a thorough stepwise technical description of this bypass, which includes a novel middle fossa craniectomy within the lateral triangle, and a modified STA harvesting. In this cadaveric dissection, we emphasize the three critical stages of this new technique: 1) STA graft harvesting, 2) middle fossa approach and IMA exposure, and 3) Exposure of M2 and anastomoses. A more proximal segment of the STA may potentially serve as a high flow conduit with a relatively larger diameter. In this way the need for more invasive techniques (e.g., ECA exposure in neck, radial artery harvest) may be eliminated. Also, with a larger diameter jump graft spanning the shorter distance between the middle fossa and MCA candelabra, the proximal MCA becomes available for bypass with potentially higher patency rates. This technique can also obviate the need for a neck dissection to expose a donor carotid in case the STA has a favorable diameter. The total length of the STA graft required to complete the bypass was 5.6cm, with a 2.2mm diameter of the distal stump. Although cadaveric dissections cannot totally simulate real life settings, this video provides detailed anatomical landmarks allowing understanding this complex technique. |
