| Video Discription | This is a revision case where a previous stapedotomy had been performed but an ongoing air bone gap persisted. The previous operation had noted a stiff malleus but basic attempts to mobilise this at that time had not been successful. Therefore in this demonstration we show a more formal division of the ossified anterior malleaolar ligament using a laser, which permitted mobilisation of the malleus without disturbing the stapes piston. The use of laser prevented any energy or movement passing through the intact ossicular chain, through to the cochlear. The endoscope gave the perfect view of the anterior malleolar ligament once the scutum had been removed. It enabled a panoramic view of the entirety of Prussak's space and allowed successful completion of this complex procedure in a transcanal fashion. Complete air-bone gap closure was achieved. |
