| Video Discription | Retrosigmoid (lateral suboccipital) craniotomy, intradural drilling of the suprameatal tubercle and microsurgical resection of a petroclival meningioma invading the cavernous sinus, meckel's cave and compressing the brainstem. Patient presented with UMNs form brainstem compression and trigeminal neuralgia. Gross total resection of the posterior fossa portion and of the portion invading meckel's cave was achieved. 1. separation of the tumor from cranial nerves III (3, oculomotor), IV (4; trochlear), V (5 trigeminal), VI (6, abducens) and VII/VIII (7/8 complex, facial, vestibular and cochlear) 2. separation of the tumor from the basilar artery and superior cerebellar artery and brainstem 3. opening of the meckel cave after drilling the suprameatal tubercle and removal of the meningioma from it Saint Alphonsus, Boise Idaho Mario Zanaty Ondrej Choutka |
