The surgery, perfomed without retroauricular incision, consisted in cleaning the mastoid cavity and all middle ear spaces. Note the ET orifice, oval window niche with the stapes footplate and erosion of the crura. Note the sinus tympani and its deepness under the facial nerve. That's why in some cases, even with an open radical microscopic mastoidectomy, there is still remaining disease. Note the deepness of the sinus tympani and the normal mucosa on it.
