The headache was mostly located over her right head. Besides, obesity and irregular MC was noted on her. Hormone profiles revealed elevation of prolactin level (39 mg/dl). She was under medical treatment by neurologist who is expert on headache treatment. Due to poor medical response and abnormal hormone, MRI was done. The images showed no pituitary tumor but a cystic lesion along the right trigeminal nerve to the posterior fossa. Right Meckel’s cave was expanded with mass effect. She was therefore transfer for surgical evaluation. Endonasal endoscopic surgery was performed. The intra-op finding revealed a cyst with clear fluid inside. Part of cyst wall was sent for pathology and revealed arachnoid cyst. The surgical steps were demonstrated in this video. The patient recovered well and there was no more headache she experienced before the surgery. |
