| Video Discription | with the widespread use of antenatal ultrasounds, hydronephrosis is picked up very early and most primary obstructive megaureters (O-MGU) would present early in life to pediatric urologist. However, in developing nations like India where such screening was not widespread in its early years many such O-MGU would go unnoticed till late when they present with symptoms. Our index patient is a 49 year old man with recurrent urinary tract infection. Evaluation revealed left hydroureteronephrosis with O-MGU with relatively poorly functioning kidney (22% on EC scan). The diameter of lower ureter was 20mm and obstructed segment was a couple of centimeters only. He underwent laparoscopic tailoring and reimplantation at our institute. Our point of technique is to leave ureter attached to bladder till all the steps of tailoring and suturing are done - this makes retraction efficient and less labor intensive facilitating tailoring and suturing. Ambidexterity is always beneficial for suturing. in case of thick walled dilated ureter, even after tailoring, One should refrain from making a very long implantation tunnel to minimize incidence of obstruction. At this age, reflux would be tolerated, but recurrence of obstruction would be devastating for the patient. |
