| Video Discription | In this video, we introduce the case of a 57-year-old female patient who presented with persistent contractions of the right facial muscles without loss of consciousness. Neurological examination revealed no sensory or motor deficits in the limbs, and cranial CT imaging suggested multiple cerebral metastases. The patient had a known peripheral lung carcinoma with lymph node metastases. Preoperative planning was performed using the Surgical Theater® software, and a microsurgical resection of a left cerebral metastasis was carried out via an osteoplastic retrosigmoid mini-craniotomy with continuous neurophysiological monitoring. Histology confirmed the metastasis as an adenocarcinoma of the lung with a therapeutically relevant ALK translocation. Postoperatively, the patient showed no new neurological deficits, and her preoperative symptoms improved.
