Common carotid artery is found beneath the neck muscles. CCA is dissected in the maximal possible course. After clamping as far apart as possible, the artery is cut. Adventitia is resected off the stumps, which are then gently dilated and flushed with heparine solution. After gentle approximation of the stumps, two stay sutures are placed on the front wall 120 degrees apart. The artery is then reversed and the third stay suture is placed in the middle of the back wall (Carre|'s Triangulation Technique). The back wall suture is completed with stitches distanced equally from each other. The front wall suture is then completed. The distal clamp is released. Some bleeding from the anastomosis usually occurs. In case of significant bleeding, the clamp is reapplied and the anastomosis is inspected for defects. In case of minor bleeding, the proximal clamp is released and the patency of the anastomosis is assesed. Videos of other microanastomosis techniques are available for free on this channel. |
