| Video Discription | 74 y.o. man presenting with cholangitis symptoms and abnormal MRI/MRCP showing what appeared to be choledocholithiasis with a stone in the mid-CBD. Initial ERCP was performed, which did not show any filling defects within the extrahepatic biliary system, however intrahepatics were mildly dilated. Subsequent imaging continued to show persistence of filling defect and repeat ERCP was performed but this time selective cannulation into the cystic duct was performed identifying the filling defect. Temporary plastic biliary stent was placed in the main extrahepatic biliary system to drain now purelent bile due to what appeared to be a case of Mirizzi's syndrome. Cholangioscopy was performed using Spyglass DS (Boston Scientific) for direct visualization and confirmation of a stone within a dilated cystic duct that had a dilated insertion into the CBD. Selective entry into the Common hepatic duct was made through a relatively smaller opening from the CBD which may have predisposed the patient to cholangitis symptoms due to a Mirizzi's presentation. |
