| Video Discription | Anterior fistula in females is classified as complex fistula due to intricate perineal anatomy, thin sphincters, and risk of higher incontinence, hence fistula is classified as complex fistula. As they are complex fistula warrants for sphincter preserving surgery. We routinely do PILTEC a modified LIFT as sphincter sphincter-preserving procedure for this complex fistula. PILTEC consists of proximal inter sphincteric fistulotomy, cauterization of deep inter sphincteric space, ligation of disconnected tract over external sphincter with vicryl 2-0 followed by curettage of distal tact. Division of internal sphincter will relieve intra-anl pressure, Cauterization of DIS will destroy glandular infection focus, Preservation of the ext sphincter: zero risk of incontinence, Curettage of the distal tract will remove infected granulation And We heal both medial & lateral woundss by secondary intention. We here in this video presents three cases of complex fistula done with PILTEC procedure with their positive outcome.
