| Video Discription | We present an anatomical and radiological study of the percutaneous balloon microcompression of the gasserian ganglion (Mullan's technique) in 5 fresh cadavers from patients who died of non neurological causes. The surgical technique, balloon's radiological appearance, anatomical correlations and results are presented and discussed. Our results indicate that Mullan's technique provides a simple, reliable, safe, cheap and effective surgical alternative to others percutaneous options as radiofrequency thermocoagulation and glycerol rhizolysis in the treatment of trigeminal neuralgia. The aim of this paper is to highlight the anatomical and radiological bases for percutaneous balloon compression in order to prevent complications during the procedure. Conclusions: Not only a trigeminal ganglion compression takes place after Mullan´s technique. After the procedure, together with neural compression other factors can influence the outcome. We think that changes in the gasserian dura stretched at the Meckel´s cave and the trigeminal porus (we call this a "dural decompression" of the trigeminal ganglion and its root), and changes in the arachnoid, as well as variations in pressure at the cerebellopontine angle cistern, may play a role on clinical results obtained with this interesting, simple and ingenious procedure for trigeminal neuralgia.
