| Video Discription | The obturator foramen bypass is an alternative procedure to solve complicated vascular problems in the groin. This extra-anatomic bypass is performed for this patient as he is suffering from radiation-induced iliofemoral arterial complications with groin radio-necrosis and excessive scar tissue in the femoral triangle. With this technique, vascular continuity is restored immediately. We expose the Distal aorta and iliac system Retroperitoneally through a Modified Rutherford-Morison incision. We utilised the Plasma-blade soft tissue dissection device to remove the stents from the aorta and iliac system. We open the ventromedial aspect of the obturator membrane by palpating the pubis and moving laterally until the tough obturator membrane is felt. The tunnel is mainly created by feel as visualization of this region is usually impossible. Scissors are used to incise the membrane over finger guidance to allow the blunt-tipped tunnelling device to traverse the membrane into the thigh. Standard above-knee popliteal artery exposure is prepared. An expanded Dyna-Flo PTFE 7 mm × 80 cm Ringed graft is tunnelled through the obturator membrane between the proximal inflow source and distal artery. The distal anastomosis is performed first to adapt the Dyna-Flo cushion to the target vessel in a standard end-to-side manner, then the proximal inflow from the endarterectomised native aorta and iliac artery is anastomosed to the Dyna-Flo graft.
