| Video Discription | In this video, Dr. Steve Sarkisian, Glaucoma Fellowship Director at the Dean McGee Eye Institute, University of Oklahoma, demonstrates how to revise/needle a bleb in an eye that has undergone prior trabeculectomy. After injecting MMC and placing a traction suture, an Alcon side-port blade is used to break all adhesions and scar tissue that has caused this bleb to fail. This type of blade is advantageous compared to using a 25 or 27 gauge needle because it will not bend and it is much sharper than a needle, so it will be more efficient and effective in achieving the goal of restored subconjunctival outflow. Moreover, this same blade can be used to make a paracentesis in the cornea and is the same blade used for making a paracentesis for cataract surgery. Therefore, it is ergo-dynamically arranged for ease of use and hand positioning. A vicryl suture must be placed in the wound after the procedure. It should be noted that the blade easily passes under the scleral flap and into the anterior chamber to restore flow. If the eye had an ExPRESS shunt, the blade could not be used in the manner, but could still pass under the flap and over the tip of the ExPRESS to restore flow, as going into the AC when needling a bleb s/p ExPRESS is neither possible nor necessary. |
