The aims of treatment are to minimize the risk of recurrence and balance the morbidity, while simultaneously preserving parenchyma to maintain pancreatic endocrine function. Central pancreatectomy is an infrequently performed procedure indicated for benign or low-grade malignant pancreatic neck tumours. This parenchyma preserving operation is associated with favourable long term exocrine and endocrine function compared to its traditional alternatives. However, the laparoscopic approach for CP remains technically challenging. Methods and Results: A 14-year-old girl presented with epigastric abdominal pain for two years. CT scan revealed a tumour in pancreatic mid-body sized 4cm x 3cm x 3.7cm. On biopsy, a diagnosis of pseudopapillary epithelial neoplasm was made. Robotic assisted central pancreatectomy and pancreato-gastrostomy was performed. The video depicts stepwise operative procedure and depicts important anatomical structures. Intraoperative blood loss was 125 ml and operating time was 190 minutes. She did not require any blood transfusion. Final pathology confirmed pseudopapillary tumour of pancreas with clear margins. She was discharged 5 days post-operatively following uneventful recovery. The patient did not develop long-term post-operative complications, including endocrine or exocrine insufficiency, and remains well on follow-up. Conclusion: Central pancreatectomy is an innovative procedure with specific applications. The robotic platform offers distinct advantages over laparoscopy with superior visualization and ergonomics which account for its increased utilization in oncologic surgery. Besides its recognized ergonomic benefits, robotics enables enhanced vision and dexterity, and allows for precise dissection around vascular and neural structures.
