| Video Discription | A case of 11-year-old female child with manifestations of chronic constipation dating since birth, abdominal pain, distention, and loss of appetite. The patient habitually passes motion once weekly induced by rectal enema. Investigations: - Barium enema: showed megarectum and megasigmoid. - Anal manometry: absent RAIR. - Rectal biopsy: hypoganglionosis with hypertrophied nerve fibers. Procedure: - The patient underwent anatomical mobilization of sigmoid colon and rectum with ligation of IMA branches. - Then the rectum was disconnected using endoGIA stapler about 3 cm above anal verge. - The specimen was extracted through right iliac incision and the mobilized colonic segment was resected. - Then, the anvil was inserted into the proximal loop, and the continuity of the colon was restored through intracorporeal anastomosis using circular stapler. - Air-leak test was done revealing integrity of the anastomosis, then silicon drain was inserted in the pelvis. - Covering loop ileostomy was fashioned with intention to perform early closure after three weeks. Postoperative outcomes: - Uneventful postoperative course. - The patient started oral fluids in the first postoperative day, semisolid food in the second postoperative day, and was discharged in the third postoperative day. |
