| Video Discription | Trans-Anal Trans-Vaginal Recto Vaginal Fistula repair , an innovative 4 Layered sphincter saving technique. Surgical Steps : 1] Evaluation of Recto Vaginal Communication. Palpate the Anal Canal and Vaginal wall with Thumb and Index Finger. 2] Gentle Debridement : Use small sized scoop to debride the Fistulous tract. Gently debride the submucosal muck from Rectal Mucosa and Vaginal Mucosa. 3] Raise 1 to 1.5Cm triangular vaginal mucosa flap on either side in such a way that they are diagonally opposite to each other with the fistulous opening at the Center. 4] Freshen the edges of exposed thinned out vaginal septum and muscularis muscle. Suture the Muscularis by using Vicryl Uro Needle. 5] Approximate the Vaginal Mucosa with the Vaginal Septum in such a way that it forms Z shape closure by using 3-0 Vicryl. 6] Raise 1 to 1.5Cm triangular Rectal mucosal flap on either side in such a way that they are diagonally opposite to each other with the fistulous opening at the Center. 7] Freshen the edges of exposed muscularis muscle. And Reinforce the Sutured Muscularis by using Vicryl Uro Needle. Make sure to protect the already sutured vaginal wall by keeping finger in the vagina at the same time. 8] Approximate the Rectal Mucosa in such a way that it forms Z shape closure by using 3-0 Vicryl. 9] Hydrogen Peroxide test to check if there is any leakage. Place a gauze soaked in Hydrogen Peroxide inside the vaginal wall and gently press it over the sutured vagina. Put Proctoscope and check if there is any leakage of Hydrogen peroxide in the rectum. Reassess and re suture if there is any leakage. Post operative instructions : Keep the patient NBM for 6hrs. Liquid diet for next 48hrs. And full diet after 48Hrs. 1st follow up at 5 days : Gentle examination to rule out any collection. 2nd Follow up after 3 weeks and 6 weeks. Join our YouTube Fam for more healing videos.
