| Video Discription | A 57-year-old female presented with multiple hepatic masses on the right lobe of the liver and one on the left lateral segment. To address all the lesions, an anatomical right hepatectomy was warranted, given that the tumor on segment V was inferior to the right pedicle. Additionally, the left lateral segment lesion would require a resection of the bulk of segment III. This would leave the patient with an inadequate FLR conformed by segments I, II, and IV. Therefore, the patient was considered a candidate for an ALPPS procedure, in which all the lesions would be addressed and growth of the FLR would be promoted. This video discusses imaging and gives a play-by-play of an open ALPPS procedure, including microwave ablation of a liver lesion. Postoperative course and pathology are also discussed.
