| Video Discription | Ventral incisional hernia (VIH) repair is commonly performed in the United States. Management of large VIH may require abdominal wall reconstruction techniques such as component separation to achieve primary fascial closure. The evolution of minimally invasive techniques using robotics has become more popular to optimize postoperative outcomes. We successfully performed a robotic retromuscular VIH repair with bilateral TAR in a morbidly obese patient with a hostile abdomen and high risk for surgical site infection. A 78-year-old man presented for elective VIH and bilateral inguinal hernia repair. His comorbidities included hypertension, diabetes, pulmonary hypertension, and prostate cancer requiring pelvic radiation. The patient underwent laparotomy in 2006 for pancreatic pseudocyst. BMI was 47.7 kg/m2. Given morbid obesity, the surgical plan was pre-operative weight loss. Unfortunately, the patient presented emergently with bilateral incarcerated inguinal hernias. He underwent open bilateral inguinal hernia repair with mesh. After recovery and weight loss to a BMI of 44.7 kg/m2, follow-up CT showed no recurrent inguinal hernias and persistent VIH. The VIH was Swiss cheese appearing with a defect 6.5cm wide, 16cm long, and a defect to rectus ratio of 1:1.8. He pursued VIH repair to avoid presenting with incarcerated VIH. Decision was made to proceed with robotic retromuscular VIH repair with bilateral TAR. Due to pulmonary hypertension, surgery was performed under low pneumoperitoneum (6-8 mmHg) and punctuated stops to allow ventilation of excess CO2. The procedure was technically challenging, but the patient did not develop surgical site infection. There are no signs of hernia recurrence to date. |
