| Video Discription | Robotic Placement of Vascular Clamps on Uterine Artery for Robotic Myomectomy Mona Orady, MD, FACOG1; Tarek Soliman MD2, 1Cleveland Clinic; 2Alexandria University Objective: The purpose of this video is to demonstrate one method of reduction of blood loss during a robotic myomectomy procedure utilizing a bulldog laparoscopic vascular clamp to temporarily occlude the uterine artery blood supply to the uterus during the procedure. Anatomy and method of application of the clamp to uterine or hypogastric arteries to minimize bleeding when dissecting the myoma from the uterine wall will be shown and proper techniques for the robotic myomectomy procedure will be discussed. Methods: This patient is a 27-year old nulliparous patient with four large uterine fibroids in the intramural location including a 9 cm lateral myoma near the uterine blood supply. In order to avoid excessive blood loss given the number, size, and location of these fibroids the decision was made to utilize bulldog clamps in order to reduce blood supply. The bulldog clamp is a laparoscopic vascular clamp that may be used to decrease blood loss during surgical procedures. Made of stainless steel or titanium, it is a spring-loaded crossover clamp with serrated blades that effectively occludes vessels without slippage or significant crush injury to the vessel. Once clamps were applied careful dissection of the fibroid utilizing hemostatic technique followed by anatomic closure of the myometrial defects utilizing barbed suture was performed. Results: Robotic myomectomy was accomplished successfully with less than 100 ml blood loss and less than 0.5 g/dL drop in Hemoglobin. Conclusions: Bulldog vascular clamps may be utilized to reduce blood loss during complex robotic myomectomy procedures. |
