| Video Discription | 42 y.o. patient. Married for 9 years, but was never able to have vaginal sex due to severe pain. Also suffers from severe pain during defecation. She had endometrioma excision and multiple myomectomy with laparotomy 3 years ago. Pelvic ultrasound examination suggested severe adhesions in the posterior compartment (absent sliding sign), bilateral hydrosalpinx, right endometrioma with a rare vascularized papillary structure, with a ROMA value of 60%. Highest I have ever seen in an endometriosis patient. Following entry through Palmer’s point, hardly found space for auxiliary trocars. Intestine, bowel and omentum were severely adhered to abdominal sidewalls, anterior wall and uterus. She needed a low anterior resection, and the largest nodule extending from one sacrouterine ligament to the other was resected. Overall it took 10,5 hours, even with Emre Balık.
