| Video Discription | #saphenousveinharvest, #patencyofsaphenousveingraft The left internal thoracic artery (LIMA) graft is the gold standard having 90–95% patency rate at 10 years, whereas only 80% of saphenous vein grafts (SVG) are patent at 10 years. With the advent of newer statins and anti-platelet agents, the patency rates of SVG's is bound to improve. To this improvement in patency rates, I guess, is the technique of harvesting the vein is also important and herewith I am demonstrating the skeletonised vein harvest technique that is free of push, pull and traction on the vein and the distention of the vein is also gentle. We have got the histopathology examination of the vein segments used for grafting and have not found any breach in the endothelial continuity or breach of the basement membrane ( electron microscopy should be used to prove or disprove this). The only disadvantage or rather difficulty of skeletonised vein is its propensity to kink if a generous length is left whilst grafting. Therefore, near perfect length matching is important for the vein to be patent and marking the anterior surface of the vein helps in overcoming kinking. Useful references are: 1.
