| Video Discription | Robotic sigmoid colectomy for diverticulitis begins with a medial to lateral approach to this colon resection. The medial mesentery is opened at the region of the Inferior mesenteric artery. The ureter is identified and preserved. The mesentery of the colon is transected in the distal direction until the recto-sigmoid junction is reached. Both right and left lateral aspects of the rectum are prepared for rectal transection. The rectum is transected with an Intuitive daVinci Sureform 60 blue stapler. Attention is turned to the proximal colon. The left colon is mobilized along the white line of toldt up to the spleen. The splenic flexure is mobilized to allow for a tension-free colo-rectal anastomosis. Indocyanine green (ICG) 7.5mg/ 3 ml is given to the patient to confirm perfusion of the colon, planned transection line and rectum. The intracorporal anastomosis is begun. A colotomy is made distal to the planned transection line. The 29mm anvil is placed into the proximal colon with a Prolene suture. The proximal colon is transected with a Sureform 60 blue stapler in the area of adequate perfusion (determined by ICG). The anvil is pulled through the staple line and the anvil tip is removed from the abdomen. The 29 mm EEA stapler is introduced into the rectum. The anvil is connected to the stapler. Orientation of proximal colon is confirmed. The stapler is fired. The integrity of the anastomosis is confirmed with an air leak test by introducing a flexible sigmoidoscope into the rectum and evaluating intra-luminal anastomosis. Of note, the pathologic specimen is removed through the right lower quadrant 12 mm trocar site using a wound protector. The peritoneum is closed as is the fascia approximated in the usual fashion. |
