| Video Discription | Shown here is a 'simplified' method of taking down the splenic flexure during laparoscopic total mesorectal excision. Step 1. Medial approach to the descending and the transverse colon starts with the identification of the IMV near the LOT. The proper plane of dissection can be recognized just underneath the IMV - Lateral endpoint: descending colon, Caudal endpoint: level of the IMA root, Cephalad endpoint: pancreas. Step 2. Supramesocolic/Anterior approach starts with the opening of the lesser sac in the direction of the spleen. The meso can now be safely dissected off the inferior border of the pancreas using the gauze as a landmark (rendezvous). Step 3. Lateral mobilization of the descending colon towards the inferior pole of the spleen and joining with the medial dissection (rendezvous). Making a 'U' turn while preserving the marginals further ensures a well-perfused tension-free colorectal anastomosis. |
