| Video Discription | This patient was referred after undergoing an emergency laparotomy for gallstone ileus and perforated bowel, requiring lysis of adhesions and bowel resection. At that time the RUQ was palpated and described as feeling like concrete. We did an ERCP, cleared the bile duct and placed a stent. A fistula to the duodenum was noted. An attempt at lithotripsy was unsuccessful thru the fistula. A CT scan showed the hepatic artery adjacent to the gallbladder. After 2 months we proceeded with robotic surgery with goal of removing the gallbladder and repairing the duodenal fistula defect. The remaining stone in the gallbladder was a potential threat of recurrence of GS ileus. |
