To minimize intraoperative blood loss and enhance visualization, the uterine myometrium was infiltrated with a dilute vasopressin solution, and a paracervical tourniquet with a Penrose drain was applied to secure the uterine blood supply. The HARMONIC scalpel HD 1000i was utilized for precise tissue dissection with minimal bleeding. Following the myomectomy, the video demonstrates the closure of a significant myometrial defect, which exposed the anterior fundal portion of the endometrial cavity. The closure process involved anchoring the STRATAFIX Symmetric suture through a two-step pass to ensure secure anchoring. The myometrium was closed in several layers using continuous, non-locking throws, highlighting the superior holding strength of the STRATAFIX Symmetric suture. Finally, the serosa was closed using a baseball-type method with alternating deep to superficial throws, ensuring well-approximated, hemostatic tissue closure with no exposed barbs. |
