| Video Discription | Endonasal and transcranial combined surgery for pituitary macroadenoma coexistent with ICA aneurysm. Because patient’s visual acuity was progressive worsen, we had no time for pretreatment of aneurysm. At first, dural ring was opened by skull base technique and cranial nerves for eye movement were identified. Cervical and distal ICA was exposed for just in case of aneurysmal rupture. Then tumor removal was performed mainly via nose and an aneurysm was exposed. As the aneurysm was observed entirely from the endonasal side, aneurysmal clipping was performed via nose. After closure with fat and fascia patch suture, nasoseptal mucosal flap was applied. |
