| Video Discription | The supracerebellar-transtentorial (SCTT) approach is a variant of the supracerebellar-infratentorial (SCIT) approach. Whereas the latter is useful for lesions in the posterolateral midbrain like cavernous malformations, the former is useful for inferomedial temporo-occipital lesions. This SCTT approach is performed with the patient in the sitting position to allow gravity retraction of the cerebellum to open the supracerebellar plane, and avoids injury to visual tracts as well as language fibers in the dominant hemisphere. SCTT approach is ideal for resection of cerebrovascular pathology such as cavernous malformations because their anatomy is relatively simple, but can also be used for medial temporo-occipital arteriovenous malformations. Ideal lesions are located above the tentorium and at, or a few millimeters beneath, the cortical surface of the medial occipital-temporal lobes. We present a video demonstrating a SCTT approach to a right medial temporo-occipital cavernous malformation in a 33-year-old male with seizures localized to the lesion, which was resected en-bloc through a small transtentorial window. The procedure was uncomplicated and the patient remains seizure-free off anticonvulsants 9 months after surgery, with no visual field deficits. Operative nuances associated with this approach are also discussed. |
