| Video Discription | Sylvian fissure split Arachnoid Dissection • Identify the Sylvian Fissure: The fissure is identified by locating its superficial landmarks. • Sharp and Blunt Dissection: Using microsurgical instruments, the arachnoid membrane over the Sylvian fissure is gently dissected. This process begins at the fissure’s lateral-most aspect and proceeds medially. • Maintain Vasculature: Care is taken to preserve the middle cerebral artery (MCA) and its branches, which run within the fissure. • Avoid Overstretching: Gentle handling ensures no traction injury to surrounding tissues. Splitting the Sylvian Fissure • Expose the Fissure: The fissure is split by carefully separating its two porcions (frontal and temporal opercula) using fine forceps and microdissectors. • Gradual Advancement: The dissection is performed in a stepwise manner, gradually advancing deeper into the fissure. • Control of Bleeding: Hemostasis is meticulously maintained throughout.
