| Video Discription | Presentation od the technique of anatomical endoscopic enucleation of prostate ( AEEP) with the hybrid Thulium YAG RevoLix HTL™ hybrid laser. Clinical case is a 70 years old man with lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH): IPSS 25 QoL 5 - Uroflow Qmax 10 ml/s PVR 70 ml. Patient is on anticouagulaiton therapy (rivaroxaban) discontinued 48 hours prior to surgery. Prostatic volume is 50 ml. Endoscope is a 26 Ch Shark™. Morcellation is performed with Piranha™ morcellator. Laser fiber is 550 micron. Laser Settings: cutting 100 W - 50 Hz - 100% effect / coagulation 5 W continuous wave or 10 W - 10 Hz - 75% effect pulsed wave Enucleation technique: 2 lobes with early apical release At the end of the procedure a 22 Ch catheter is placed with the balloon at 50 ml in the bladder for 48 hours. Anticoagulation therapy starts after 1 week. 6000 IU LMW Heparin is used at the postoperative period for 1 week. The advantages of the laser is the precision of dissection and the excellent hemostasis. The settings utilized for cutting allow a no-contact dissection, reducing charing and improving visualization of the surgical plane. |
