| Video Discription | Repair of groin hernias remains one of the most common operations performed in the United States every year. In the uncomplicated setting, laparoscopic repair is preferred over open for a variety of reasons (1). The two most common laparoscopic techniques for inguinal hernia repair are the totally extraperitoneal (TEP) and transabdominal preperitoneal (TAPP) approaches. Debate exists on the pros and cons of each approach. In a 2005 Cochrane review, TAPP was found to be associated with a higher risk of port site hernias and visceral injuries while TEP was associated with more conversions to an open procedure (2). A 2012 randomized trial comparing TAPP to TEP groin hernia repairs found TEP to be associated with less pain and higher patient satisfaction (3). The TEP approach to the inguinal hernia repair, until now, was thought only able to be easily and reproducible accomplished using laparoscopic techniques. However, with the use of the da Vinci Xi® Surgical System, we are on the forefront of this pioneering technical approach, namely the robotic TEP IHR. We are now regularly, and for some surgeons nearly exclusively, performing TEP IHRs as our minimally invasive technique of choice. We believe the procedure is easily teachable, has an abbreviated learning curve and is less expensive and quicker to perform than robotic TAPP and laparoscopic TEP procedures. Additionally the robotic TEP IHR protects patients from the notorious complications inherent to robotic TAPPs while maximizing the advantages of robotic surgery. 1. Daes J.
