| Video Discription | Welcome to Dr. Fan of Ears. For the best viewing experience, change your settings to 1080p HD. In this video, I perform a transmastoid CSF leak repair on a left ear. A postauricular incision and mastoidectomy have already been performed. The site of the CSF leak at the tegmen has been identified. 0:15 - A 3 diamond burr is used to further expose the site of the CSF leak. 0:20 - A black handle instrument is used to develop a plane between the tegmen and dura. 0:25 - The tegmen defect is further widened to facilitate the placement of a dural substitute and a hydroxyapatite plate. 0:33 - Bipolar cautery set at 5 watts can be used to obtain hemostasis on the dura. 0:39 - The dural substitute is slid into place between the middle cranial fossa dura and tegmen. 0:46 - Then the hydroxyapatite plate is slid into place under the dural substitute. 0:51 - A second plate is slid into place 0:55 - Finally, a third plate is slid into place 0:58 - A large piece of gelfoam is placed over the ossicles to protect them from the bone cement 1:02 - Bone cement is applied to the mastoid cavity to seal the defect and to prevent further CSF leak. Cottonoids and pressure are applied for 5 minutes as the cement hardens.
