| Video Discription | This is the case of a 30 year-old male patient with 8 year-history of seizures and mild cognitive impairment. Imaging studies showed a lesion within the anterior segment of the medial temporal lobe involving the amygdala and hippocampal head. A selective microsurgical resection via transsylvian approach was selected to maximize preservation of dominant temporal pole and anterior temporal cortex and underlying fiber tracts, such as inferior fronto-occipital, inferior longitudinal and optic radiations. In this case, the Sylvian fissure opening was not easy as there were several large superficial Sylvian veins that had to be microdissected to gain access to the deep Sylvian cistern and media-basal temporal lobe. Multiple technical and anatomical nuances are described in this surgical video. |
