| Video Discription | The endoscopic Draf III procedure (or endoscopic modified Lothrop) has been highlighted in other videos on this YouTube channel, but this was a nice case highlighting the benefits of using a mucosal graft to cover the exposed bone of the nasofrontal beak after the Draf III procedure. The Draf III procedure results in the widest opening possible into the frontal sinuses. By removing the middle turbinates, superior nasal septum, intersinus septum, and nasofrontal beak, the Draf III procedure turns two separate frontal sinuses into one common frontal sinus drainage pathway (https://www.youtube.com/watch?v=9Vj7YdrD5ng&list=PLmNtAozOCdnt41482_kSNwbMimuGA3w3n). While many Draf III cavities remain patent without placing a mucosal graft over the exposed bone of the nasofrontal beak, some will scar down significantly, and it has been shown that mucosal grafting to this area decreases the risk of scarring and failure of the Draf III operation. The patient in this case example had a complex history after having undergone an endoscopic resection of a sinus cancer followed by radiation therapy. This treatment put the patient at significant risk for sinus scarring, and the left frontal sinus scarred off, and a mucus collection built up in the sinus that required surgical drainage (“mucocele”). The most reliable frontal sinus drainage path that can be created surgically is with the Draf III procedure, but because of his radiation history, it was felt that his exposed bone would be especially problematic in terms of healing, and the bone could even become infected. Therefore, a nasal floor mucosal graft was harvested and placed over the exposed bone of the nasofrontal beak, and it healed beautifully, and the frontal sinus cavity remained widely patent at 3 months postoperatively.
