| Video Discription | Introduction: This video is showing few steps of Laparoscopic surgical procedures to support the vaginal vault to minimise the chances of vault prolapse. The incidence of vault prolapse is less than 1% but it seems to be increasing nowadays.During hysterectomy procedure in benign conditions if the surgeon put some attention in few specific points during vault closure which may help to bring down the incidence further low. Variety of procedures offered to those patients & there is chances of recurrence.Preventive steps during primary procedure may definitely help those who are at risk. A 60 yrs lady presented with complaints of something coming down p/v since few years . On examination it is found that the lady has elongation of the cervix which is coming out of the vagina without cystocele & rectocele .There is laxity of the vagina.After counselling she is posted for TLH.During the procedure 3 steps are performed as a prophylactic steps to prevent vault prolapse later on. (1) Uterosacral ligaments are cut above the level of there attachments in continuity with the posterior vaginal margin (2) Attention is given during vault closure to fix & tightening the ligaments securely to the vault (3) A non-absorbable suture material [ Ethibond 2-0] used through ligaments, vaginal vault for further reinforcement .Care should be taken to prevent accidental injuries to the ureters when taking deep bites in uterosacral ligaments . |
