Here we demonstrate vaginal morcellation of a globular fibroid after TLH. It is necessary that the cervix descends up to the introitus after TLH. Now the specimen can be converted into a narrower strip as follows: Protect bladder anteriorly and the rectum posteriorly by inserting two wide Sim's speculums. Cut on your right side from 12 o'clock to 6 o'clock, exerting gentle traction on the cervix. Hold the apex at 12 o'clock with an Allis or vulsellum. Remove the anterior speculum, rotate the specimen clockwise so that the earlier 12 o'clock now becomes 6 o'clock. Repeat the previous steps progressively climbing higher.
