| Video Discription | Laparoscopic ventral mesh rectopexy : The American Society of Colon and Rectal Surgeons guidelines endorse ventral mesh rectopexy as a strong option for rectal prolapse repair, citing its acceptable short- and long-term complication rates and reduced risk of postoperative constipation compared to posterior approaches. Key steps: Limited anterior rectal mobilization: The rectum is dissected anteriorly from the vagina (in women) or prostate (in men) down to the perineal body, avoiding posterior and lateral dissection to preserve autonomic nerves and minimize postoperative constipation. Opening the rectovaginal (or rectovesical) septum: The peritoneum is incised from the sacral promontory to the pelvic floor, exposing the anterior rectal wall. Inverted J fashion incision. Mesh placement: A synthetic or biologic mesh is sutured to the ventral (anterior) aspect of the distal rectum with non-absorbable sutures at the site of intussusception. The sutures should not go into rectal mucosa. Rectal resuspension: The proximal end of the mesh is anchored to the sacral promontory, typically with tacks and sutures, ensuring care to avoid nerve injury. The mesh is fixed to the sacral promontory, resuspending the rectum and correcting the prolapse. Peritoneal closure: The peritoneum is closed over the mesh to reduce the risk of mesh-related complications. The repair is checked for tension and position. Outcomes: LVR is associated with significant improvement in constipation and incontinence, low recurrence rates (typically 3–10%), and a low incidence of mesh-related complications. Mesh erosion and fistula are rare but recognized risks. Long-term patient satisfaction is generally high, though new pelvic pain or sexual dysfunction may occur in a minority of cases. Recurrence of prolapse is a recognized long-term risk, with rates around 8–12% at 10 years.
