| Video Discription | (Language: English/French). Right revision otosclerosis surgery following previous surgery with prosthesis extrusion. Transcanal approach. Middle ear exploration reveals a simultaneous malleus and incus erosion. Ossiculoplasty is performed with a Grace Vincent HA-Teflon TORP positioned from malleus to staedotomy (malleus-to-stapedotomy procedure). The major problem in this case is related to the eroded malleus as the residual malleus handle is too short for a secured and stable TORP's head placement. This video shows a technique of trimming the malleus neck using a 0.7mm diameter diamond dust burr to artificially increase the length of the malleus handle thus allowing for a malleus-to-stapedotomy procedure. Alternative option would be to insert a Malleus Replacement Prosthesis (MRP) as shown in previous videos of this channel. The labyrinth is fenestrated and a vein graft is interposed.
