| Video Discription | This video shows a typical case of minipterional or sphenoid ridge keyhole approach for clipping of unruptured middle cerebral artery bifurcation small aneurysm. Preoperative angiography and vessel wall MRI demonstrated a MCA aneurysm as unstable because of bleb and perianeurysmal strong enhancement. However, operation revealed that the aneurysm had two lobes composed of fibrotic thick lateral dome and true aneurysm on medial side. The pathologic report described the medial thick walled aneurysm had smooth muscle cell hyperplasia with cuboidal morphological change and infiltration of macrophages with formation of microvessels. |
