| Video Discription | Onodi cell is an important anatomical landmark within the confines of the posterior ethmoid air cells. Sometimes the Onodi cell is markedly developed and extends above and lateral to the sphenoid sinus in a degree that will make identification of the sphenoid sinus during endoscopic sinus surgery a difficult task. If the Onodi cell is markedly developed the optic nerve and the carotid artery will bulge within the lateral wall of the Onodi cell making endoscopic sinus surgery dangerous surgery if care is not taken in identification of this anatomical variation during the preoperative evaluation of the CT scan in the different planes. Kindly remember this statement while doing endoscopic sinus surgery, the level of the sphenoid sinus ostium is at the same level of the upper border of the maxillary sinus. If you open a cavity within the posterior ethmoid and you are hesitating whether this is an Onodi cell or the sphenoid sinus just look at the upper border of the maxillary sinus ostium and you can identify where is the sphenoid sinus ostium. |
