The deliberate staging of surgery may be considered for large brain arteriovenous malformations. It is necessary to deconstruct the arterial pedicles while preserving adequate venous drainage from the nidus. With very large AVMs access to arterial pedicles may be impeded by large arterialised veins necessitating their strategic deconstruction while ensuring the balance between arterial irrigation and venous drainage is maintained.
