| Video Discription | This video forms part of a series that support the Royal College of Surgeons England Neurovascular Fellowship at King's College Hospital, London. A 29 year old male presented with a WFNS grade 1 subarachnoid haemorrhage in a non-perimesencephalic distribution. Initial CT angiography identified two areas of concern which may have represented infundibula. Digital subtraction angiography confirmed the presence of an infundibulum at the origin of the right posterior communicating artery. There remained uncertainty about an abnormality on the dorsal proximal A1 segment of the anterior cerebral artery. Follow-up digital subtraction angiography demonstrated a change in morphology in this lesion, confirming an aneurysm. The aneurysm measured 2 x 1 mm. Multidisciplinary review by interventional neurologists and neurovascular surgeon judged it was unsuitable for bare coiling. Placement of a flow diverting stent to reconstruct the diseased vessel was not felt optimal given the need to administer anti-platelet therapy after the recent haemorrhage. Surgical repair was recommended and carried out via a lateral supraorbital mini-craniotomy. Blister aneurysms are typically small and arise on the dorsal aspect of vessel sidewalls. They are notoriously friable and intra-procedural rupture is frequent. Once ruptured reconstruction is difficult and wrapping, suture or vessel bypass may be required. Direct clip repair- with or without cotton reinforcement- is often feasible although preparations are also made for wrapping with woven cardiovascular fabric, direct microsuture or vessel bypass.
