| Video Discription | Calcification is common in Craniopharyhgiomas. In order to achieve total excision it is mandatory to excise the whole tumor along with all the ‘calcium’ as well. Often this is amorphous and can be excised in piecemeal manner as shown. The is grasped with the micro biopsy forceps and counter traction is applied by a ‘Zero’ micro suction so that any chance of traction on or injury to the perforators stuck to the calcification (tumor) are reduced; however tempting it is to remove the calcification ‘en bloc’ one must refrain from this and remove it in piecemeal manner. This allows better visualization of the perforators stuck to the far side of the tumor and prevents possible injury to surrounding structures and perforators when a large bit of calcification is yanked out The short video clip is from an old recording on a one chip camera and the surgery is a left fronto temporal craniotomy, there are two self retaining retractors one on the frontal brain and the other on the temporal; the sylvian fissure has been opened and the dissection is lateral to the carotid which is an avenue I like to use whenever possible. |
