| Video Discription | This 24-year-old man had met with a vehicular accident in his hometown; he had suffered severe fracture injuries of his right side upper jaw, cheek bone and eye socket bone. He was treated in some other hospital elsewhere for his injuries. Now he reported to our hospital complaining of inability to close his right eye and dryness of eye. His right eye was at a lower level than his left eye – this affected his appearance. He had ectropion of the right eye-a condition in which the eyelid is turned away from the eyeball, and enophthalmous – sunken position of eyeball within the eye socket. Also he had an improper bite of his upper and lower jaw on the right side. CT scanning of his facial bones showed incorrectly joined fracture of the cheek bone & upper jaw (zygomatic complex) and the fractured floor of the right eye socket bone was sunken. A complete reconstruction surgery was done. A rib graft and a fat graft were taken, a valsalva maneuver was done to rule our injury to the lung tissue and the incision was closed in layers. The cheek bone region (zygoma) on the right side was approached from inside the mouth. The incorrectly fixed bone plate in the previous surgery done elsewhere was removed. The displaced and malunited zygomatic arch was re-fractured, pushed back and stabilized. The graft was fixed to this depressed zygomatic region to augment it and to elevate and reconstruct the floor of the right eye socket. Lateral canthotomy and cantholysis was done for right eye. The skin was removed from the fat graft, the fat was tunneled into the lower eyelid and anchored to the medial canthus. Lateral tarsal strip was anchored to the lateral wall using sutures. The surgical site was closed. Following successful surgery, he was able to close his eye completely and his jaw alignment and bite was greatly improved.
