| Video Discription | Surgical Date: 2013-10-11 Surgical Team: IgorTFerreira + Daniel Marzullo Video shows a laparoscopic approach of a appendicectomy for an appendicitis with perforation of the base. Access trhough 10mm port on the umbilicus, insertion of 2 5mm ports on the left side of the patient (on left iliac fossa an left flank). In the inspection, that was noted a blockade of epiplon and terminal ileum at the right iliac fossa. After some blunt dissection the appendix was identified attached to the the peritoneum of the inguinal region, with necrosis of the tip . The mobilization of the tip was followed by the cauterization of the mesoapendix with the use of monopolar Hook (40w cut + 30w coagulation - Blend 2 settings). Close to the base that was identified a localized abcess that spills some pus. On the base that was found a perforation and a fecalith. The fecalith was removed to an extraction glove, After the confirmation of the isolation of the base (with good tissue below the perforation), the proximal base was ligated with 2 endoloops of prolene zero (hand-made ones) and another one distal. The section the the base was performed with scissor, the appendix was removed to the inside of the glove, and the glove was closed. Irrigation and aspiration of all the quadrants were performed (with the help of an additional 5mm port on the righ hipocondrium(. The glove was extracted through the umbilical 10mm port. No drain was used. The aponeurosis of the umbilicus was closed with prolene zero suture and the skin of all 4 incisions sutured with mononylon 4.0. Good post-operatory evolution, patient released 2 days after the surgery, with oral antibiotics for 7 more days (ciprofloxacin + metronidazol). Patient presented a good evolution, no complications observed.
