| Video Discription | This 58-year old woman had been suffering from long-standing volatile blood pressure fluctuations with systolic blood pressure peaks up to 280 mmHg and diastolic blood pressure nadirs of 40-50 mmHg, refractory to medication. After a rigorous diagnostic trajectory hypertension of neurogenic origin was suspected. MR imaging showed compression of the left ventrolateral medulla oblongata and root entry zones of cranial nerves IX and X due to a dolichoectatic left vertebral artery. We performed a microvascular decompression via a retrosigmoid approach with a good outcome at 4 months (the time of this surgical movie post), with stabile blood pressures albeit high, for which antihypertensives were administered. |
