| Video Discription | A 64-year old lady presented with a Fisher grade II subarachnoid haemorrhage (SAH). CT-angiography and conventional angiography showed a giant ophthalmic aneurysm, that was not eligible for endovascular treatment. One day after the SAH the aneurysm was therefore treated with microvascular techniques. Via a left-sided pretemporal transcavernous approach and cleaning of the distal dural ring the neck of the aneurysm was exposed. Remodelling of the neck and clipping of the aneurysm resulted in complete obliteration of the aneurysm at angiographic control. Surgery by: dr. E.J. Delwel and dr. R.
