| Video Discription | #DrVishalSoni #gallbladder #gallstones #SafeLaparoscopicCholecystectomy History: 64 yr Male, DM, HTM, Parkinsons, Obese with history of progressive pain since 7 days with associated fever. USG/CT showed Calculous Cholecystitis with stone/s impacted at neck. CBD was clear. Mildly derranged LFT Setup & Steps: Entry with Verees, 4 ports used 10mm * 2, 5mm *2, HD system with 30 degree Camera. Diagnostic Laparoscopy showed Gangrenous Changes in the Gall bladder. Calots found to be frozen. Decision taken to proceed with Subtotal Cholecystectomy. Transaction done in mid-portion. Gallbladder examined and transaction done further down. Stones impacted at neck disimpacted with repeated gentle suction-irrigation & extraction with grasper. Stump closure done with 3-0 V-Loc continuous suturing. Bile leak wasnt seen. Specimen extracted with Endobag. Washes given. JP Suction drain placed. 10 mm defects closed with Vicryl no 1.] Post-op: Unevetful recovery. No Bile leak. Discharged on POD3. Drain removal on POD5. Labs & USG were normal.
