| Video Discription | In cases of indirect inguinal hernias with long and difficult-to-reduce sacs, a laparoscopic approach was performed to repair a right inguinal hernia using the hernia sac abandonment technique. This strategy is particularly useful in patients with a large hernia sac adherent to the spermatic cord, where complete dissection could increase the risk of bleeding or injury to the vas deferens. 📍 The technique consists of reducing the herniated contents while leaving the distal sac within the scrotum, after safely dividing its base and adequately covering the area with a mesh placed in the preperitoneal space. 📍 In this case, a conventional mesh with fixation was used, ensuring stable coverage of the myopectineal region and preventing displacement. The sac abandonment technique is safe, evidence-based, and can prevent unnecessary complications in selected cases. Recent studies demonstrate similar rates of seroma, hematoma, fluid collection, and recurrence compared to traditional full dissection.
