| Video Discription | Recurrence of hernia after laparoscopic repair is usually dealt with by open repair. However, experienced lap hernia surgeons can manage the problem by re-laparoscopic approach. We describe a case of a patient operated elsewhere by laparoscopy for hernia and developed a recurrence of the issue. Patient preferred a repair by laparoscopic technique. We do a TAPP approach for these patients. Points to take care off: 1. The peritoneal incision is made proximal to the cranial edge of the previous mesh. 2. The mesh is taken down along with the lower peritoneal flap. 3. One has exercise extreme caution in the region of the external iliacs because of the adhesion of the previous mesh to the vessel adventitia and peri-adventitial tissue. In case of dense adhesion, the previous mesh is left on the vessels. |
