On oto-endoscopy there was TM perforation and PTA showed Conductive hearing los. Rest of ENT was unremarkable Plan: Rt Modified Butterfly Tympanoplasty Procedure: After adequate patient counseling following established aseptic otologic protocol Transmeatal approach was adopted Perforation margins were freshened. Tragal Cartilage graft was harvested and fashioned as modified butterfly in which a strip of cartilage is left on central part, while rest of cartilage s removed and perchondrium on the upper surface is kept intact.A grove is made at both ends of cartilage with overlying perochondrium.Thus the free ends of cartilage are inserted under the perforation margins while overlying perichondrium covers the perforation.Canal was filled with gelfoam and polyfax pack.Pt was instructed regarding post-op care and follow up visits. |
